Passport and Consular Miscellaneous
Please note: For all Passport related services, applicant should come in person.

Following services are available:
 

1. Issue/Re-issue of Passport.
2. Renewal of a Short Validity Passport.
3. Issue of duplicate passport (in lieu of lost or damaged passports).
4. Issue of a new passport booklet due to:

a). Change of name

b). Change of appearance

c). Change of Surname 

d). Change of date of birth or place etc.

5. Issue of Emergency Travel Documents.

Re-issue of a  Passport

An application for re-issue of passport can be made before SIX months of expiry of the old passport. If pages in the passport are exhausted, applicant should apply for re-issue.

Requirements for re-issue of  Passport:

1. Duly completed application form filled in BLOCK LETTERS in expanded form using black or blue ink only.
2. Four latest, identical PP size (4 X 4 Cms) photographs depicting the applicant's front pose in sharp focus against light background. The application should be duly signed with signatures of size 1.5 X 4.5 cms within the box as specimen signature.
3. Where the Passport has already expired for more than one year, the applicant should submit an affidavit.
4. Vietnamese Work permit/Residence permit/Letter from Employer.
5. Current passport.
6. Fee (see schedule).
7. Applicants applying for re-issue of passport are required to submit a photocopy of marriage certificate, in case the name of spouse has not been included in the passports.

Passport for a minor

Please note:

• Every child should have a separate passport. The practice of endorsing the child's name on mother's passport has been done away with.

• The validity of such a passport (for children under 18 years) will be for 5 years only.

• If the child’s name was endorsed on the mother's passport, then the mother also has to fill in a misc. services form requesting deletion of a child’s name from her passport.

The applicant should complete the formalities as applicable for the re-issue of passport as mentioned above. The child has to sign or put a thumb impression (as applicable) on the application form which.

Documents to be submitted

1. Duly completed application form filled in BLOCK LETTERS in expanded form using black or blue ink only. Application should be duly counter-signed by the parents or legal guardian.
2. Four latest, identical PP size (4 X 4 cms) photographs depicting the applicant's front pose in sharp focus against light background. The application should be duly signed with signature of size 1.5 X 4.5 cms or thumb impression within the box as specimen signature.
3. Original birth certificate (duly attested) and parents' passports with a photocopy of each.
4. A declaration is to be submitted duly signed by the parents.
3. Fee (see schedule).

Duplicate passports (in lieu of lost or damaged passports).


The application is to be submitted with the following documents:
1. “Application form” (one copy).
2. Copy of police report with English translation.
3. Six PP size photographs. ( 4 x 4 Cms)
4. Fee (see fee schedule).
5. Affidavit.
6. Photocopy of the previous (lost) passport.
7. Photocopy of Vietnamese Residence permit, Driving Licence, Employers letter, etc &
8. Application on plain paper giving reasons for the loss of passport.

Change of name /surname

1. The change has to be first advertised both in India (in an English language daily circulated in the area of your permanent residence and in and English language daily of Vietnam).
2. The original newspaper clippings.
3. Court Order issued by a judicial officer not below the rank of a FIRST CLASS MAGISTRATE in India OR an Affidavit by the applicant. (process for the issue of a new Passport will be followed).

Change of name / surname after marriage.

(This requires the issue of a new passport booklet )

1. Duly completed “Application for an Indian Passport”.
2. Original & Photocopy of Marriage / Divorce Certificate.
3. Fee (see fee schedule).
4. Photocopy of husband's passport should be submitted if wife’s name is already endorsed. Otherwise, husband should also follow the same procedure simultaneously. In case either of the spouse is a foreign national' only photocopies of Marriage certificate and foreign passport should be submitted with the application form.
Change in appearance

This facility can be utilized where  there is a significant difference in the appearance of the applicant in the passport (Infant/Child, Turban and Beard etc) In case of turban, non-turban and vice versa, an affidavit must be submitted. (The process for the issue of a new passport booklet will be followed )

Change in date of birth / place of birth

Allowed only on the production of a court order issued by a judicial officer not below the rank of FIRST CLASS JUDICIAL MAGISTRATE in India

Emergency travel document

An Emergency Certificate (one-way travel document) which authorizes an Indian citizen to enter India, is issued to individuals who have lost their passports or their passports have been stolen or damaged and to whom new passports cannot be issued without approval from India.

This document is issued primarily with the objective of ensuring the applicant's return to India in an emergency. The applicant is not allowed to travel to any third country on an Emergency Certificate. “Application Form for an Indian Passport” is to be used.
